Summary Statement of Benefits

These benefits may change at any time at the discretion of management.
Medical Coverage
Full Time employees and their qualified dependents become eligible on their date of hire.
Harvard Pilgrim Health Care HMO
- Provides comprehensive in network care
- Members must select a primary care physician
- Referrals to a specialist are required
- $10 office visit co-pay
Participants will make a pre-tax paycheck contribution towards insurance costs, according to family status.
Dental: Delta Dental Premier
- Within the extensive network:
- Every 6 months - preventative - 100%
- Basic restoration - 80% after deductible
- Major restoration - 50% after deductible
- $1,500 calendar year maximum

Domestic Partner Coverage
Domestic partners are eligible for coverage under the medical and dental insurance plans.
Life Insurance
Marathon will provide one times your annual salary in term life insurance.
Optional Life Insurance
Supplemental coverage is available in multiples of annual salary up to five times, to a maximum of $500,000. Premiums are based on age and the supplemental amount selected. Premiums are paid through payroll deduction. Spouse and dependent coverage is available.
Short Term Disability
Marathon provides short-term disability insurance for the period not covered by the LTD plan below. After an elimination period of 7 calendar days, this insurance provides 67% of regular base salary (plus an average of commissions, if applicable) for up to 90 days of a disability.
Long Term Disability
Marathon offers employees the option of purchasing long-term disability insurance, through payroll deduction. The benefit is 60% of base salary (plus an average of commissions, if applicable), to a maximum of $10,000/month, for employees who are disabled for more than 90 days.
Accidental Death & Dismemberment
Basic coverage provided by Marathon is equal to the employee's base salary.
401(k) Plan
Marathon offers participation in a 401(k) plan with Scudder Investments. Employees become eligible on the first of the month following their hire date.
Section 125 (Healthcare Flex Spending Account)
Employees may have withheld pre-tax, up to $3,000 for out of pocket medical/dental expenses.
Vacation
Regular full-time employees receive 15 days each calendar year (accrued monthly).
Sick Time
Regular full-time employees receive 5 sick days per calendar year to a maximum accrual of 10 days.
Holidays
Marathon provides 11 paid holidays in each calendar year.
Skills Training
Marathon provides selected training programs for employees when required for job performance.
Section 125 Plan
Employee paid contributions towards medical, dental, and 401(k) are withheld on a pre-tax basis.
Employee Referral Program
Marathon's employee referral program provides employees with a $3,000 bonus if a candidate they refer is hired.
